If you do not have insurance coverage, we accept self-payment (credit card, debit card, or Zelle by using newsunrisemh@gmail.com). However, if you do have insurance that we do not cover, we are happy to provide you with a statement ("SUPERBILL") that you can submit to your insurance company for reimbursement. If you choose to use your out-of-network benefits from your insurance, it’s important that you call them directly to verify that you do in fact have out-of-network benefits. Please contact them before setting up your first appointment.

  • New patients

    • Initial evaluation in person or video (telemedicine) appointment (up to 60 min): $400 
    • Please complete required electronic Intake Form before the appointment.
    • Get $100 discount for Initial Evaluation

  • Established patients/follow-up appointments

    • Currently, we have the choice of in-person (Boca Raton office), telephone or video appointments

  • Cost for other services

    Documentation completion, such as letters or any legal or other form completion, will incur a $200.00 fee.


    Sending medical records and handling fees: $35.00 for the first 50 pages and 10 cents for each additional page thereafter.

    Please understand that appointment times are limited. If you must cancel your appointment,

    we respectfully request that you do so in a timely manner.


    A cancellation is considered late when the appointment is cancelled less than 24 hours before

  • Appointments

    You can request an appointment by sending us a confidential email using the appointment request form or by leaving a message on our confidential voicemail at (561) 944-0077. Appointments start promptly at the scheduled time. Late arrivals may lose appointment times and will either need to reschedule or be seen after other patients. Punctual arrivals have priority.

  • Cancellation Policy

  • Termination of Service

    Multiple late cancellations, no-shows, and other forms of noncompliance with treatment may result in the termination of services.

  • Medication Refills

    Patients may need to schedule an office appointment within 3 months from the last visit to obtain medication refills. This is a requirement for patients on some controlled substances.

  • PCI Policy for the Protection of Cardholder Data

    Any cardholder data provided to New Sunrise Mental Health by its patients is subject to this PCI Policy.
