Fees & Insurance Coverage



  • TRICARE (Humana Military - East Region)

  • Florida Blue (Effective July 21st, 2024)

  • AETNA 

  • UNITED HEALTHCARE (OPTUM: Oscar, AVMED Florida Exchange, Florida Exchange, UBH General).

If you do not have insurance coverage, we accept self-pay (credit card, debit card, or Zelle by using However, if you do have insurance that we do not cover, we are happy to provide you with a statement ("SUPERBILL") that you can submit to your insurance company for reimbursement. If you choose to use your out-of-network benefits from your insurance, it’s important that you call them directly to verify that you do in fact have out-of-network benefits. Please contact them before setting up your first appointment.



New Patients

  • Initial evaluation in person or video (telemedicine) appointment (up to 60 min): $400 

  • Please complete required electronic Intake Form before the appointment.

Established Patients / Follow-Up Appointments

  • Currently have the choice of in person (Boca Raton office), telephone or video appointments

  • Medication management appointment (up to 30 min):  $200



Documentation completion, such as letters, any legal or other form completion will incur a $200.00 fee.


Sending medical records / Handling fees $ 35.00 for first 50 pages and 10 cents for each additional page thereafter.


Appointment Cancellation Policy

Please understand that appointment times are limited. If you must cancel your appointment,
we respectfully request to do so in a timely manner.

A cancellation is considered late when the appointment is cancelled less than 24 hours before
the appointed time. A no-show is when a patient misses an appointment without cancelling. In
either case, we will charge the patient a $50 fee.
