Patient Forms
New patients are required to fill out a number of forms. It is important that you complete this step before your first appointment, as the forms contain important information required for a proper diagnosis. Please allow for enough time prior to your first appointment to complete this documentation.
At New Sunrise Mental Health, all paperwork is filled out online through our patient portal. It is not necessary for you to print, scan, or email any documents.
New patient forms fall in one of two categories:

Intake forms - these forms ask for information about your medical & psychiatric history as well as your family's. Please fill this out to the best of your ability. They include:

Consent forms - these are forms usually required by law in order to be treated by a physician. Please review them and sign them electronically prior to the start of your first appointment. They include:

Treatment agreement, in which you review and accept the policies of New Sunrise Mental Health.

Telehealth consent form, in which you agree to be treated via secure online video or by phone.

Notice of privacy practices, required by law.

Email communication consent, in case you choose to communicate with Dr. Ibarra-Alos outside of our secure patient portal.
